1. All deposits are NON refundable.
   
   
2. Company is not liable in the event of mechanical breakdown while on charter and will only be responsible for making up lost time at a mutually agreed date. In the event the contracted vehicle experiances mechanical problem, accidents, overbooked by our sub office, or has been sold before the contracted service date, Five Star Enterprise & Limousine has the right to substitue a like vehicle or vehicle with adequate seating capacity and Five Star Enterprise & Limousine will not be held responsible to compensate customer otherwise. We reserve the right to cancel and refuse service and return the deposit.
   
   
3. Drugs and Weapons are strictly prohibited, and the law prohibits anyone under the age of 21 to consume alcohol beverages in the vehicle.
   
   
4. SMOKING IS STRICTLY PROHIBITED IN OUR VEHICLES. The customer will be charged $350.00 in the event any attempt to smoke in the vehicle.
   
   
5. The customer will be charged and billed for any damages to the vehicle such as cigarette burns, broken glass, broken stereo, cosmetic damages, ect.
   
   
6. A minimum charge of $300.00 will be accessed and charged to the customer's credit card for detailing of the vehicle in the event that there are excessive trashing and human waste.
   
   
7. Violent behavior and or Slander will automatically terminate your service: there are no refinds of full payment and/or deposit.
   
   
8. The client assumes full financial liability for any damage to the limousine caused during the duration of the rental by them or any members of their party. Any fines will be paid for by the customer. The driver has the right to terminate run without refund (if there is blatant indiscretion on the part of the client(s)). It is Illegal to stand through the sunroof.
   
   
9. Overtime pay will apply after the first 15 minutes of prearranged time described on the run sheet.
   
   
10. Not responsible for delays or the termination in winter caused by unsafe road conditions (ie. not salted, accidents, etc.).
    
    
11. Not responsible for articles left in the limousine.
    
    
12. Vehicles cannot be loaded beyond seating capacity.

 

All charges, fees and discounts are estimated based on standard rates and your account settings. Additional tolls, parking and other fees may apply. Actual total amount due will be calculated upon completion of the trip. Should you have any questions, please contact us at 678-616-4135.

 

By submitting this form you agree that (1) you are the credit card holder and (2) that you are requesting the services listed above and (3) that you are authorizing Five Star Enterprise & Limousine to charge the credit card listed above for the requested services.

 

All confirmed reservations will be charged a 20% Non-Refundable Deposit. Full payment and ballance must be paid in full one week prior to the schedule pick up date. All payments made one week prior to scheduled pick up date is Non-Refundable Deposit. We reserve the right to cancel and refuse service and return the deposit.
